About the job

As a Nurse Practitioner, you will provide primary care to senior residents, enhancing their health and quality of life. Working within an interdisciplinary team, you’ll perform assessments, develop care plans, and educate residents and families about treatment options.

Key Responsibilities
  • Conduct physical exams and diagnose acute and chronic conditions
  • Develop and implement individualized care plans
  • Prescribe medications, order tests, and perform medical procedures
  • Collaborate with physicians, nurses, and other staff
  • Educate residents and families on health conditions
  • Maintain accurate EHR documentation
Qualifications
  • Active Nurse Practitioner license with a geriatric focus
  • Minimum 2 years’ clinical experience in geriatrics
  • Excellent assessment and communication skills
